Fee schedules

MPFS, OPPS, DMEPOS, and CLFS lookups — pick the schedule that matches the setting

Facility-aware estimates (GPCI, form, POS) use POST /v1/knowledge/payment-calc. Use /v1/fee-schedules when you already know which CMS schedule applies and want the raw row.
